6) Vessel’s eta RIZHAO(船舶的预到时间与日期)

7) How many crew have valid yellow fever vaccination/cholera vaccination cert./China Health Cert.(检疫与身体信息)

8) Which port do you supply provisions & fresh water last time(最近一次伙食与饮用水供应信息)

9) Is there any dangerous cargo o/b?(危险品货物信息)

2，DOCUMENTS REQUIREMENT BEFORE VESSEL’S ARRIVAL FOR VESSEL’S REGISTRY

1) Last ten port of call list(挂靠港口的历史记录)

2) Arrival ship’s particular【FORM A, see attachment1】

3) Arrival crew list【Note: Crew list should be 100% correct w/o any errors. Otherwise ship wl be fined RMB10,000 by immigration.准确性提示 FORM IMO, see attachment2】

4) Arrival crew effect declaration【FORM IMO, see attachment 3】

5) Arrival ship’s store declaration【FORM IMO, see attachment4】

6) Arrival ship’s certificates scanned copy【Note: Refer to the attachment5, all nominated files/certs should be scanned copy and send as PDF documents】

3，Declare to port VTS upon arrival RIZHAO port limit

1) Ensure yr vessel’s AIS system should be in good working condition.【打开并确保船舶的AIS系统正常工作】

2) Ensure report to VTS on CH 16，and then stand by on CH1,10,23,26.upon vessel’s arrival

6，SAFETY NOTICE FOR VESSEL’S ARRIVAL

1) RIZHAO port NAVIGATION SAFETY NOTICE issued by port MSA for yr perusal【See attachment6 】

2) Ensure to report to VTS once yr vessel anchor down at nominated anchorage position.

3) Ensure to keep watch on bridge and deck upon yr yr vessel’s stay at anchorage at anytime. It is possible for anchor’s turning upon wind scale is over L7

二） BERTHING AT RIZHAO PORT

1，Keep touch with yr charterer’s agent to obtain updated berthing schedule day by day.

2，Reconfirm the fixed pilot schedule once receiving before anchor up if occurring difficult weather condition including big wind/big wave/dense fog/etc...

3，Pls ensure to fix the pilot boarding point with pilot station, RIZHAO pilot point asf:

4，Ensure vessel’s pilot’s ladder in good/secure condition, please sign and send back the safety notice of pilot ladder【See attachment7】

5，Please prepare following documents with ship’s stamp and yr signature for vessel’s entry formalities:

① Crew list: 4pcs

② Crew effects declaration: 2pcs

③ Ship’s store declaration: 2pcs

④ Ship’s particulars(FORM A):1pc

6，Please collect all crew’s passports(seaman’s book)according to yr record in crew list and get ready o/b

③ Any drills/hot work/painting/ect...should obtain permission before conducting

④ Pay attention to the correctness/accurateness of crew list

⑤ Alcohol/Drug(narcotics)/Smuggle sale/smoking is forbidden at port

1.1	拟使用的引航梯是哪一年制造的？ What is the manufacture date of the pilot ladder that you intend to use?		年份 year

1.2	引航梯是否有符合IMO Resolution A.1045(27)附录第2条引航梯规定的制造商证书？ Are the certificates issued by the manufacturer of the pilot ladder were available on board ship in accordance with IMO Resolution A.1045(27) Appendix ,Article 2.?		Yes 口	No 口
1.3	贵船是否对拟使用的引航梯在使用前进行适当的外观检查，确保其踏板、边索和扶手绳没有可辨识的损坏，所有踏板无油漆、弄脏或湿滑的？ Has inspection been carried out for pilot ladder before use to ensure that there is no damage in the steps, side ropes, man ropes? The steps should not be painted, dirty or slippery.		Yes 口	No 口
2.0	引航员登离船的安全管理 Safety Management of Pilot Boarding Operation
2.1	IMO关于引航员登船装置要求的贴示图是否张贴在贵船驾驶台？ Is the poster of Required Boarding Arrangements for Pilot by IMO posted on the bridge?		Yes 口	No 口

2.2	贵公司的SMS是否有关于引航员登离船操作的作业安全指导书？ Is the safety working instruction for pilot transfer operation included into the company's SMS?		Yes 口	No 口
2.3	贵船相关船员是否知晓上述规定、与引航员登离船操作相关的风险及针对性防范措施？ Whether related crew are well aware of above regulations,and also familiar with the risks relating to the pilot boarding operation and targeted preventive measures?		Yes 口	No 口
3.0	安装引航梯须知 Notice of Rigging the Pilot Ladder
3.1	派专职驾驶员在现场监督引航梯的安装。 Designate a officer to supervise the rigging of pilot ladder on scene.
3.2	用绳索或卸扣将引航梯与船体系牢。 The pilot ladder is securely fastened with ropes or shackles to the ship.
3.3	引航梯的安装与IMO发布的引航员登船装置要求贴示图一致。 The pilot ladder is rigged in accordance with the poster of Required Boarding Arrangements for Pilot issued by IMO.

3.4	相关安全设施配齐可用（无线电话、救生圈、撇缆绳）。 The appropriate safety gears (radio, lifebuoy, heaving line) must be ready for using.

4.0	试踩试登引航梯注意事项 Notice of Having a Try on Pilot Latter
4.1	船方应安排船员（穿着救生衣和系好安全绳）试踩试登引航梯； Please arrange a crew(wearing lifevest and safety line) to have a try on the pilot ladder；
4.2	如果仅使用引航梯，试踩试登船员需要完整攀爬引航梯一次； If only using pilot ladder，the crew who is to try on the pilot ladder should climb the pilot ladder all over one time;
4.3	如使用组合梯，试踩试登船员需要通过组合梯，至引航梯攀爬一次； If using ship's combination ladder, the crew who is to try on the pilot ladder should walk through the combination ladder，then climb the pilot ladder all over one time;
4..4	引航员在海上登离轮之前，船方船员试踩试登引航梯作业必须在现场引航员的监督下执行, 否则引航员有权拒绝登离轮。 Before pilot embarking/disembarking at sea,the trial on pilot ladder must be carried out under the supervision of the pilot on scene(within sight),otherwise the pilot has the right to refuse to embark/disembark the ship.

8，Prior to sailing, the boarding Agent will collect all necessary papers and obtain clearance from Immigration, Customs and Harbour Master, and provide the vessel with port clearance.
